"Did my husband receive the best care?"

About: Angliss Hospital Peninsula Private Hospital

(as a carer),

• My husband had been receiving ongoing treatment at Peninsula Private Hospital. One day while at home, he was not well and I was advised to take him to the hospital.

• While my husband was getting ready to go he fell over and I was unable to lift him, this resulted in me calling for an ambulance. The ambulance staff were going to take my husband to Peninsula Private but were advised to take him to William Angliss as Peninsula Private did not have a room available.

• Paramedics took my husband to the Emergency Department and he was well looked after by a doctor and a very competent nurse. When a bed became available he was taken up to the ward, this is where my husband's problems began. They appeared to be overworked and understaffed, I felt very sorry for the staff, however, one person, I believe they may have been a junior doctor, tried several times to insert a line into my husband's arm, eventually the junior doctor got it in and then realised that they had put in into a blood vessel not a vein and seemed very concerned. The junior doctor then attempted to put a line into my husband's right hand and again tried several times before they decided to call for the assistance of a senior doctor. The junior doctor asked me to put pressure on the wad of cotton wool they had put there while they found the doctor.

• The senior doctor arrived and had to re do all the work that this junior doctor had previously done. My poor husband's hand by this stage looked terrible, in a very short time it had swollen up, it was very red and was twice its normal size. That night he developed a high temperature.

• The next day my husband was transferred to Peninsula Private, the nurse in charge there took one look at his hand and was very concerned; she even took a photo and swabs of the hand because she did not want Peninsula Private to be held responsible for the condition.

• Constantly they reduced my husband's temperature each time up it increased again by the end of the day.

• It was stated that my husband had Sepsis showers as a result of the hand.

• Within a week my husband was clearly not well and was transferred to Intensive Care, they tried so hard to get my husband's condition under control but to no avail, two days later all the vital organs had shut down and he passed away.

• Why was a person who did not appear to be competent in inserting lines etc allowed to attend to my husband? I believe this resulted in bacteria entering his body that he was unable to fight.

First of all, please allow me to express my condolences to you and your family following the passing of your husband. You must be utterly devastated and I am really sorry to read that you believe poor care at the Angliss Hospital may have contributed to his death.

As I am sure you will understand, this is also of great concern to me and I would like to take the opportunity to review this, understand what occurred and to be able to provide you with further information. Sharing stories through Patient Opinion allows us to understand where we do things well and also, where we need to improve and I am keen to improve this situation if I possibly can. From the details you have provided, I am unable to identify any details directly about your husband's care but if you write to me directly at alan.lilly@easternhealth.org.au, I will arrange a review as a matter of priority.

Once again, I am so sorry that we have caused so much concern for you and I hope that you will contact us so that we can undertake a detailed review.

In the meantime, I will discuss this incident with the Executive Director in charge of the Angliss Hospital and alert him to the concerns which you have raised.
